Encephalisation really only occurs in bilaterally symmetrical animals. Whole phyla like Cnidarians (jellyfish, anemones etc) and Echinoderms (starfish, urchins etc) are headless and even some bilaterally symmetrical animals like bivalve molluscs are also headless. Even animals with a distinct front and rear like nematodes or worms lack heads because while they might lead with a mouth they lack the concentration of specialised sensory organs and brains that truely define a head.
