1. The veterinary surgeon seems to be very tactful, as he convinced Mrs. Pumphrey that Tricki was really sick, just so that he could get Tricki to exercise and get fit. He knew that Mrs. Pumphrey would never be able to stop feeding Tricki, so he tricked her for the betterment of Tricki. HE indeed uses a lot of common sense in order to help the dog.
2. Tricki was extremely happy to go back to his mistress. Mrs. Pumphrey might've understood that Tricki was getting sick just because of her over-feeding Tricki. She might stop stuffing Tricki with food, and might even let him run around for a bit of exercise.
3. This seems to be a mixture of both, since the problem Tricki had is something a lot of people/animals suffer with nowadays. But there is also a bit of fiction, since most veterinarian surgeons will not go out of their way and lie just for the betterment of a dog. They would also not eat all the food given to a pet.
