Social equality and harmony in a community are indeed important, in order to maintain an individual’s health and vice-versa. After all, the health of the whole community depends on the personal habits of various individuals who constitute the community.

There’s no hard reason why someone needs to be seen as equal to have good health. There have been times where being overweight was a sign of wealth and people would take care of slaves health to ensure they could work for longer.
That said, it’s more likely that privileged class get more health benifits and poor are left to suffer. So while not necessary, it is more likely.
