Tocainide, a structural analog of lidocaine, is a Class IB antiarrhythmic agent. Tocainide was developed specifically to reduce first-pass metabolism to enhance oral activity. The electrophysiological actions of tocainide are similar to those of lidocaine. While tremor and nausea are the major dose-related adverse effects, tocainide is rarely used because it can cause bone marrow aplasias and pulmonary toxicities The use of tocainide is generally restricted to life-threatening ventricular arrhythmias that do not respond to other therapies Hutchison and Shahan (2004), Roden (2001), Sweetman (2004).
