It is advised not to touch the bulb of thermometer while reading the measure because the bulb of thermometer comes in contact with the body of the patient and raises the level of mercury. If it is holded from the bulb then the measure would change and correct measurement can not be taken.

The bulb of thermometer is where you touch the surface on to get the temperature. Holding the thermometer by the bulb will cause the mercury to move because the hand also has body heat. So then you wont be able to get the correct temperature as the mercury will not be in the correct place.
