While Holmes and Smith were discussing, Watson overheard them from the next room. As they were discussing, Smith confessed that he had killed his nephew, Victor and also sent an infected ivory box to kill Holmes in the same way. Holmes asked Smith to turn up the gas light which was a signal for Inspector Morton.

Culverton tries to kill Holmes too by sending a box that is designed to infect him with the same tropical disease. Due to lack of evidence, Holmes decides to trick the murderer into confessing by pretending that he is in mortal danger. As Holmes had expected, the arrogant Smith confesses to killing his nephew as well as his (attempted) murder of the detective himself.
