Although the S(+)-enantiomer is capable of inhibiting cyclooxygenase (COX) at clinically relevant concentrations, R(-)-ibuprofen is not a COX inhibitor.
Only one of the isomers is effective because it is the one that fits the receptor site on the enzyme involved in pain perception.
