u= - ∞
For first refraction at A
μg/V-μa/u= μg-μa/R1
1.5/V-0=1.5/10
v=30cm
for second refraction at B
u= 30-5=25cm (virtual object)
R2= - 10cm
1/v-15/25= -0.5/-10
v= 9.1cm
So the image is formed 9.1cm from the Second surface of lens
