Subject: Leave Application

Respected Madam

It is to inform you that I am a student of your class and I was not able to attend the school for the last two weeks. This was because I had to attend my sisters' marriage in Jaipur. Due to some family reasons, we had to pre-pone some arrangements and I had to rush there without formal any intimation.

I request you to please accept my leave application for the last two weeks. I promise I will cover up all the lessons I missed and this will not affect my performance. I will be highly obliged.
