Sir,
I'm sorry to have to disturb and inform you that I lost my briefcase on day before yesterday while travelling a crowded bus on my way to home. There was a cheque book in it with two unused leaves. The leaves contain my name - Trisha Roy and account no. xxxx68869.
I have already lodged a diary to the officer in charge Entally Police Station about the loss of brief case, cheque book and other items. I am enclosing a copy of the diary as ready reference.
In case, someone should attempt to use to lost cheque. You are requested to take necessary action that I may not been put in trouble.
