It is given that ,

Slope = -1/4

Point = (-5 , 4)

We know that , the point slope form is given by

 \large \sf \underline{ \:  \fbox{ (y -  y_{1}) = m(x-  x_{1})} \: }

Substitute the values , we obtain

 \sf \implies (y - 4) = -   \frac{1}{4} (x - ( - 5)) \\  \\\sf \implies 4(y - 4) =  -  \frac{1}{4} (x + 4) \\  \\\sf \implies 4y - 16 =  - x - 4 \\  \\\sf \implies  x + 4y - 12 = 0

Thus , the equation of the line is x + 4y - 12 = 0
