there is two questions in your one question

(a) The person is suffering from both myopia and hypermetropia. because The range of normal eye vision is 25cm to infinite. Hence the distance is 25cm to 300cm, I guess the person os suffering from both eye defects.

(b) The person requires bi-focal lenses to increase his range of vision. The upper part of the bi-focal lens is a concave lens which facilitates distant vision while the lower part is a convex lens which provides near vision
