Antacid pills usually contain bases or alkaline substances inside a capsule. The capsule when reaches the stomach opens due to stomach conditions and the base reacts with some of the acid molecules releasing salt and water. Bases have a pH higher than 7 so the stomach pH is reduced. Thus there is lesser irritation on the stomach walls due to high acid concentration. The person's uneasiness decreases gradually after the pill has started to function.
