(i) Diabetes mellitus
(ii) Insulin

(b) Pancreas. Its function is to secrete enzyme - Pancreatic Juice and hormones - Insulin,Glucagon,Somatostatin.

(c) When the hormone (insulin) is secreted in less amount than required,then,blood-sugar level rises and a person may have Hyperglycemia (Diabetes mellitus). Similarly,when the hormone is secreted in more amount than required,then,blood-sugar level rises and a person may have Hypoglycemia.

(a) Smita's father is suffering from the disease Diabetes mellitus and the hormone whose deficiency causes it is Insulin.

(b) The gland that secretes it is Pancreas and this hormone(Insulin) functions to maintain the blood-sugar level.

(c) When the amount of insulin secreted is less,then,the blood-sugar level increases leading to Hyperglycemia(Diabetes mellitus) and when the amount of insulin secreted is more,then,the blood-sugar level decreases leading to Hypoglycemia
