(a) Difference: Evaporation is a surface phenomenon; it takes place at all temperatures. Boiling is a bulk phenomenon. It takes place at fixed temperature.
Similarity: Both process takes heat from the surrounding.
(b) Chemical name of dry ice is solid CO2. It just looks like ice but is absolutely different - Co2 is taken in an enclosed cylinder and compressed by applying pressure. The product obtained is dry ice.
