This is to bring to your attention that I had purchased and ordered a TV set (Order details and order no.) from your esteemed shop, but  it was delivered in a damaged condition at (time & place). The frontal cover and outer buttons were completely damaged. Therefore kindly replace the faulty item with new item or refund the money.
