After the general assurance of getting the american prisoner slain, the doctor should have felt relieved,but he spends sleepless nights how do you account for his behavior ?

Dr. Sadao was not only a great doctor, but also a great human being. He had been trained to save lives, not to kill them or see them killed. With Tom getting washed ashore near their house, he was trapped in a conflicting situation. Despite so much of stress and pressure, he instinctively made life-saving decisions for the POW.

When he confided to General Takima, the General planned to have the POW quietly killed to save Dr. Sadao from the consequences. According to the plan, General’s assassins were to come to Dr. Sadao’s house and kill the POW in his room and dispose of the body. Dr. Sadao remained extremely tense for a couple of nights as the assassins didn’t turn up as per the plan. So, each night he lay awake thinking about the poor POW, who could be killed any moment. His nights were tense and sleepless.  

This tells he was a good soul to the core.
