In case of plants, carbon dioxide gives C and water is the main source of O. N is taken from the roots of the plant which get deposited in the soil as nitrates by the process of nitrogen fixation.

In case of animals, they usually enter the body through food. As food chain starts from the plants, all the nutrients including C, N and O gets transferred to the other eaters.
