In the actual story by o Henry, behrman dies of pneumonia.

johnsy is saved because of the hope the hypothetical last leaf instilled in her. He hears the story about how he saved her after his death. She informs this to the people around and they feel really proud of behrman.

Behrman braves the rain for painting a leaf on the wall which looks so realistic that johnny falls prey to the deception. Though behrman has an attack of pneumonia, doctors are able to save him and he becomes very famous for the leaf he painted.
