As per mirror formula we know that

\frac{1}{d_i} + \frac{1}{d_o} = \frac{1}{f}

here we know that

d_i = 37.5 cm

f = 15 cm

now we have

\frac{1}{37.5} + \frac{1}{d_o} = \frac{1}{15}

d_o = 25 cm

so distance of object from the mirror is 25 cm

Now from formula of magnification we have

M = \frac{d_i}{d_o}

\frac{6cm}{h_o} = \frac{37.5}{25}

h_o = 4 cm
