Apology to your English teacher for having misbehaved in English period

Dear Ma'am,

I am ashamed of my self that what I have done in the class today. I fully understand that my behavior wasn't good at all and it was a disgraceful act by me. I am aware of the results of my today's act that I will never get success with disrespecting and disobeying you.  

I am very sorry for misbehaving you. I beg your forgiveness. please forgive me for my mistake. I promise you that this will never happen again.
