Subject: Leave application for one day due to fever

Dear sir,

It is respectfully submitted that I'm Rithu Varma working as Team Leader under your command. I wish to inform you that I have developed symptoms of fever last night. I visited hospital last night for treatment. The doctor prescribed medications for my quick recovery and recommended to take rest for one day.

In this regard, I'm unable to come to office for today(dd.mm.yyyy). So I'm submitting this leave request to you for one day of sick leave. I'm hoping to recover my health soon. On account of my unwell health, kindly please allow me one day leave.
