Sir/Madam,
With all due respect it is to state that I am a student of (Department and Subject name; example: MBA Marketing) in this university/College. I am a good reputed student and worked hard in my educational career and I am very punctual too. The college/university issued Identity cards to students for security reason. I was also given such card but because of loose cord I lost my card yesterday (or few days ago). Kindly issue me the duplicate of the said card as soon as possible.
I will pay the extra charges for the issuance of the card. Thank you very much.
