Subject: Application for five days leave.

Respected Sir,

Most humbly I beg to state that my uncle is sick and undergoing medical treatment at the Government Hospital of my village. So, I would like to pay him a visit him and take care of him for a few days.

Therefore I earnestly request you to grant me leave for four days with effect from 14th to 18th November 2019 along with station leave permission.

I will always remain grateful to you for your kind and sympathetic consideration of my application.
