Sir,

It is to inform you that I have lost my bag yesterday in the bus(no.WB02 6021) I was travelling and my library card(no. 9876543210) was inside the bag. I searched for the bag a number of times but being unsuccessful, I went to the local Police station and lodged a General Dairy.

I shall be glad if you please issue a duplicate library card. I will bear all responsibilities for the books I borrowed with the lost card.

I attach the photocopy of GD along with the photocopy of my lost library card.
