Dear Mrs. Mayer,

SUB: REQUEST FOR SICK LEAVE

I am writing this letter to request for a sick leave that will take four days according to the doctor’s note attached below. I am a senior class 4 student and I suffer from asthma and a condition called Conjunctiva Rhinitis. It has been uncomfortable for me to make it to school on time because of the winter season that is aggravating my situation.

I am kindly requesting to be granted four days of leave to see my doctor and begin medication immediately so that it doesn’t affect my studies. I would prefer to start my leave on Thursday and end it on Sunday to avoid missing a lot on school work.
