Respected Sir,

Most humbly I beg to state that my uncle who is living in Bareilly is seriously ill and undergoing medical treatment at a hospital. So, my entire family would like to pay him a visit him in Bareilly and take care of him in the hospital for some days.

Therefore I earnestly request you to grant me leave for one week from 14th to 20th November 2019 along with out of station permission.

I will always remain grateful to you for your kind and sympathetic consideration of my application.
