You need to write a letter to the Manager of the branch where the FD is located, requesting him to transfer the FD Account. I would just write a proforma letter.

__________(write branch name)
__________(write place - Pin code)
Sir,
I am holding an Fixed Deposit account with number ______________(write the account number of the FD. Like your SB Acc, your FD will also have an Account Number) at your branch. I would request you to transfer the FD account to the following branch :
State Bank of India
__________(write the transferee branch name)
__________(write the branch code here)
Thank You
Yours faithfully,
_________(your name and signature).
The above one is a draft. Just copy the letter on a A4 size white paper. Write date and place at the top right corner. Fill up the blanks with appropriate data. Don’t write the content written in the brackets. Submit the Letter to the branch manager, along with your FD Bond. The FD Account should be transferred with in 2 or 3 working days.
