No, all elements except for the Noble gases are not  diatomic molecules.

Eg: Carbon (C), Lithium (Li), Sodium (Na), etc.

Metals are not diatomic ( S block and some of p block) .Many d and f block elements like uranium , copper are not diatomic

No it is possible that under extreme conditions of high temperature and pressure bond cleavage ir breakage occurs.

So answer for your question is normally almost every element exists in diatomic state but in extreme conditions bond breaks and the following molecule becomes monoatomic.
