No, square roots of some positive integers are rational too.

for example:-

√4 = 2

and 2 is a rational no.

No, the square roots of all positive integers are not completely irrational, but mostly. The square root of all primes are irrational.

Ex - 4, 9, 25, 16, 121, 625.
