as a 12 year child you don't need any king of diet chart but still if you want than here is is.Basically it's not a diet chart what you need to do is just eat less of fats and oily things eat more of roughage and fire nad water containing fruits and vegetables like cucumber,orange,pomegranates,apple,etc.eat less of rice,bananas and potatoes as they increase your body weight
