Explanation:

When cleaning of tanks (Carbaryl plants, Bhopal) was in progress, water entered badly maintained Methyl isocyanate tanks. This caused chemical reaction resulting in carbon dioxide built over other things too.

As the pressure became high, the tank valves had been triggered in emergency to reduce the pressure of 200 °C. But the rusty pipes made the reaction faster resulting in leakage of poisonous gases into the environment.
