SIMPLE PAST:
He broke the vase.
The vase was broken by him.

PAST CONTINUOUS:
He was checking the copies last night.
The copies were being checked by him last night.

PAST PERFECT:
I had written this letter.
The letter had been written by me.


SIMPLE FUTURE:
I will write a book.
A book will be written by me.

FUTURE CONTINUOUS:
I will be writing a book soon.
According to the rules, it cannot be converted to passive voice. So no change occurs.

FUTURE PERFECT:
I shall have read this book.
This book shall have been read by me.
