The formulation of voice from active to passive is :-

Object + auxiliary verb + verb in past participle + subject.

Q :- Have you finished the report?

Ans :- Has the report been finished by you ?

REMEMBER :- The tense never changes in voice .

As in the question there is have so we have to add been with it.
