the sentence 'You do not help me.' is simple present and is in negative form because it has 'not'.

Active form of sentence  

⇒ Subject + do/does + not + first form of the verb + object

The subject ⇒ you

Verb ⇒ help

Object ⇒ me

So, The Passive form of the sentence will be  

⇒ Object + is/am/are + not + third form of the verb + by + subject
