The given sentence in passive voice can be written as:

Is my umbrella wanted by you? Let it be taken.

In active voice, focus is on the subject whereas in passive voice the focus lies on the work done, and so the subject becomes the object and vice-versa.

The sentence ' take it' is an imperative one.Such sentences when converted into the passive voice , start with 'Let'.

The given sentence can be converted into passive as shown below:

If my umbrellas is wanted by you it can be taken.

or

Is my umbrella wanted by you? You are requested to take it.

The first sentence was in simple present tense; we used the passive form of the same. The second sentence was an imperative sentence; so we used the imperative form of the passive.  

Imperative sentence is a sentence expressing an order or request or advice.
