Respected sir/mam,

Myself XYZ (write your name) and this letter are to inform you of a very poor health service in my society/area and the reason is because of lots dust, garbage and dirt the rate of mosquitoes in my area is increasing and diseases like dengue and malaria are taking place in the large amount.

To make the things more worse we are not even having a good hospital and medical stores near us.

And therefore there is no proper health care...

I hope that you'll look after my concern because if it takes too long then the things will start getting more worse.
