Given Three Eye defects  and its correct match :

Farsightedness          Presbyopia                           Near nearsightedness

Hypermetropia            Problem of old age                   Myopia
 
Convex lens                 Bifocal lens                         Concave lens

Explanation:

Myopia is also called as Near nearsightedness that means a person suffering with this eye defect can see near things clearly but finds difficultly in far things.
So its corrected by Concave lens.

Farsightedness is also called as Hypermetropia .A person suffering with this eye defects finds difficulty in seeing near things .
So it is corrected by Convex lens.

Due to ageing and weakening of Ciliary muscles , old age people find difficult in near and far vision . So it is corrected by Bifocal lens where upper part is concave and bottom part is convex.
