putting 

−3=rcosθ,0=rsinθ

9+0=r2(sin2θ+cos2θ)

r2=9

r=3

rcosθrsinθ=−30

tanθ=0

θ=0

So, ∣z∣=3, argument of z=0

Therefore,  polar form of z is 

z=r(cosθ+isinθ)

=3(cos0∘+isin0∘)
