Steps to add form button:

•Click the Customize Quick Access Toolbardrop-down arrow.

•Select More Commands.

•In the Excel Options dialog box on the Quick Access Toolbar tab, from the Choose commands from drop-down list, select Macros.

•Select the macro you want to add to the Quick Access Toolbar and click Add and then OK.

• Add the Form button to the Quick Access Toolbar

•Click the arrow next to the Quick Access Toolbar, and then click More Commands.

•In the Choose commands from box, click All Commands.

•In the list box, select the Form button and then click Add.
