A true hermaphrodite is someone who has an intersex condition named ovotestis. This sexual developmental disorder is characterized by one of the three following situations:

the presence of gonads that contain both testiculair and ovarian tissue [mixed gonad]

the presence of one testicle and one ovarian;

the presence of one mixed gonad and a ovarian/testicle.

So, there are variations. A hermaphrodite is not someone with both a functional penis and vagina. Ambigious genitalia can be present, but this is not always the case. The prader scale explains more about ambigious genitalia.

Karotype can be normal of mosaic. There is often onset of breast grow and menstruation in puberty even when the hermaprodite was raised as male. This is often the cause of the discovery.
