Health- speaks for the physical (or even mental) fitness. Whether your body (or mind) works the way it should in the given circumstances.
Well-being on the other hand is the status of your body (or mind).
Eg: "He was healthy, but died from depression."
This shows how when we speak about health its only the physical entity, whereas well-being is the state of mind or your body (exhausted, euphoric, excited, etc.)
2.Application.
Health is applied to basically medical conversations, where right Blood-pressure, heart rate or cholesterol levels might predict you a longer life.
Well-being is applied to monetary, for-their-own-good topics and also emotional wellness.
