From the relation we can get that the osmostic pressure = concentration*Reidbergconstant(R)*temperature.

And entirely the osmotic pressure depends on the temperature and the concentration of the solution.

Hence, for a solution to be isotonic the concentration must be same throughout and the solute to the solvent ratio must be constant so that the net osmotic pressure remains same.
