When injury occurs,platelets rupture to release thromboplastin. Thromboplastin acts on prothrombin in the presence of calcium ions(Ca+). Thromboplastin converts prothrombin into thrombin. Now, this thrombin acts on fibrinogen and forms fibrin. Fibrin undergoes polymerisation to for a chain or network of fibrin. It is like a wire mesh that covers the reptured cappilaries and injured skin. This mesh entraps blood corpuscles this disallowing the cells to go out. This forms a jelly like mass which we call a blood clot. At last a pale yellow coloured serum is released!
