First of all, "Agile" and "Waterfall" are not really methodologies - many people make the mistake of thinking that they are, and assuming that there is a binary and mutually-exclusive choice between the two. Those two terms are very widely misused in practice..."Agile" is not a methodology - it is a general adaptive and iterative approach for managing projects based on the principles and values defined in the Agile Manifesto. The term "Waterfall" is also widely-misused in practice - it might be used for any plan-driven methodology rather than the true definition of "Waterfall" which was a specific methodology created by Dr. Winston Royce in the 1970's that really isn't widely-used any more at all.

If you're looking for a "cookbook" solution, you're probably not going to find that, in my opinion. There are many alternatives between what people call "Agile" and "Waterfall"; but, it takes some skill to know how to blend those two approaches in the right proportions to fit a given situation. In today's world, projects don't necessarily all fall into nice, neat, "cookbook" methodologies - the bar is being raised higher for a project manager. A project manager today has to be able to know how to blend the principles and practices behind "Agile" with the principles and practices behind traditional, plan-driven project management in the right proportions to fit a given situation. It's something like the difference between being a "cook" and a "chef".

Some of the distinct differences are: Agile is an incremental and iterative approach; Waterfall is a linear and sequential approach. Agile separates a project into sprints; Waterfall divides a project into phases. Agile helps complete many small projects; Waterfall helps complete one single project.
