Explanation:
In Providencia, Chile's Bustamante Community Park Tower B on June 8, 2014, a ThyssenKrupp elevator unexpectedly broke down, rose 30 floors in 15 seconds while the doors were still open, and then slammed into the roof of the shaft. Serious injuries to a man's head and legs were reported, along with other injuries. The elevator was thought to have free-rolled in the opposite direction because it was thought that the brake had been picked (opened) and there had been a loss of motor control. The elevator also lacked a rope brake and an overspeed governor, which prevented unintended car movement and ascending car overspeed, respectively. As a result, the elevator "fell" upward because the counterweight is heavier than the elevator cab.
