This form is only valid for account holder who intend to receive the payments by himself/ herself.
A withdrawal form is a slip which is printed on a piece of paper that is used by the bank to allow the account holder to withdraw cash from his/her account. The account holder has to fill the slip which contains certain details relating to his/her account like date, name of customer, amount to be withdrawn in both words and figure and the signature of account holder etc.
