The conventional definition -- the polarizability of the medium -- would put vacuum permittivity at zero, so that's not what it actually means. The actual meaning is hidden in quantum electrodynamics as essentially the number of virtual photons that come out of a charge.

You might consider the permittivity as the sum of the vacuum permittivity plus the polarizability of the medium, or 1 + chi.

In this context, a negative permittivity means that when you apply a field, the medium polarizes opposite to the direction of excitation. And this happens quite frequently. It either requires use of a conductor, where it happens up until the frequency hits the plasma frequency, or a material with a very strong resonant frequency, where it happens near the resonant frequency.
