Focus is generally synonymous with hypocenter. So depth is included. Epicenters are the projection to the Earth surface of the hypocenter or focus.
It should be recognized that all three terms correspond to a point source idealization of a planar or 3D slip event. Usually they refer to the location of the onset, but they could refer to some other place, such as a place of major slip, which radiates more energy.
