Not necessarily. You can even change the state just by changing the temperature.
For example, boiling water by raising the temperature till its boiling point or reducing the pressure till such extent that it boils at room temperature.

Yes,every time we have to make a change in the temperature and pressure to change the state of a matter. Because even at a particular temperature a substance may not change its state if pressure is not enough and vice versa.
Example-
Water boils at 99.6°C at 1ATM(atmospheric pressure) but it can boil at lower temperatures at low pressures (at mountains). Therefore pressure and temperature are the important factors in cahnge in state of matter.
